Registration Information for Orthotic, Prosthetic, OrthoSEAT, and Assistive Technology Services

Appointments
Most patients are seen by appointment. Please call to schedule an appointment to ensure that you receive personal care and attention when you come to our office. Walk-in appointments are honored on a first-come, first-served basis. If you cannot keep an appointment, please give 24-hour advance notification so care can be provided to another patient during that time.

Prescriptions
We provide Orthotic, Prosthetic, OrthoSEAT, and Assistive Technology services based on the prescription or referral received from your physician. Most insurance companies require that Mary Free Bed Orthotics & Prosthetics keep a prescription on file to receive payment for services rendered. Prescriptions may be faxed or mailed to our office.

Insurance
Because your insurance policy is a contract between you and the insurance company, we ask that you verify your benefits for services. We will obtain prior-authorizations for services rendered and will submit claims to your insurance company. Mary Free Bed O&P will attempt to appeal insurance rejections to receive proper payment based on your insurance policy. If we are unsuccessful in obtaining full payment for services from your insurance provider, the final responsibility for payment remains with the patient .

You may be asked to sign a waiver if an item or service is not a payable benefit by your insurance company. This paperwork will be handled before services are rendered.

Monthly payment plans and a charitable funding program are offered to help alleviate patients' financial burden should your insurance not make full payment. 

Patient Preregistration Form
Please download and print the preregistration form, fill it out as completely as possible, and bring it with you to your first scheduled appointment. This will expedite the check-in process.
